The Patients page is your list of patient records. Linking an encounter to a patient lets Autochart.ai use their details and past visits to write more accurate notes, and lets you schedule visits ahead of time.
Plan availability: Patient Management is included on the Intensive plan or higher. On lower plans it is available as an add-on. See pricing and plans.

The Patients page only appears when Patient Management is turned on. See How to Turn On Patient Management.
Each row shows the patient's name and age, number of encounters, date of last visit, date of birth, phone number and MRN (Medical Record Number).
Troubleshooting
- The Patients page is missing: Turn on Patient Management in Settings -> Patient Management. Your plan or add-ons need to include it.
- I cannot save a patient: Check that First Name, Last Name and Date of Birth are filled in.
- I cannot find a patient: Check the spelling and clear the search bar. Patients are listed under the name they were saved with.
- Duplicate patients: Two patients can share a name. Use the date of birth, phone and MRN shown on each row to tell them apart.
- Patient data is protected: Details are stored securely. To control how long records are kept, see Data Retention.
