Your Note: Your AI-generated Medical Chart

In this article, we’ll explore the AI-generated medical note, how to copy it into your EMR, and how to regenerate it with updated settings.

Once you complete an interaction, it appears in the Encounters View. Clicking on a specific encounter opens the Documents View, where the Note View on the left displays the AI-generated medical note. This note has been generated based on the settings you chose on the Recording Window (e.g., Specialty, Prompt, Input Language, Note Type, Brevity, and Output Language).

At the top of the Note, you’ll find the title, date, start time, end time, and duration. The title is editable—just click on it to make changes.

The medical note is fully editable—click anywhere to start typing, and your changes will auto-save.

Once you’re satisfied with the note and ready to act on it, you have the following options:

  • Copy Rich Text: A formatted version with headings, bullet points, and spacing for a clean, professional layout—ideal for EMRs that support rich formatting.

  • Copy Simple Text: Plain text without formatting, suitable for EMRs or text boxes that don’t support rich text.

  • Send by Email: Email the note directly to yourself.

  • Archive or Delete: Archive to save the note for later retrieval, or delete permanently.

Tip: Depending on your EMR, try both Copy Rich Text and Copy Simple Text to see which integrates better, as the output’s appearance depends on the formatting supported by the receiving text box.

CAUTION: Be cautious with delete, as permanently deleted charts cannot be recovered. Additionally, charts are stored in our system for a maximum of 30 days for privacy reasons. We strongly recommend transferring records to your EMR as soon as possible.

Pro Tip:

Want to copy section by section? Simply hover your cursor near any heading until it changes to a hand icon. Click the heading, and that section will be copied to your clipboard. You can then paste it directly into your EMR.

Regenerating Notes:

If a generated note doesn’t meet your needs, click Regenerate to adjust the settings, add more information, upload files, images, etc. and much more. For example, in the Regenerate Window, you you can provide additional instructions (e.g., lab results or medical history), change output formats (e.g., SBARQ), change brevity or output language. You can also change your Specialty and Visit Type or prompt to reflect your encounter more accurately. Click Regenerate Note to get the new output

Resume Feature:

To start recording again for a generated note, click Resume. The system will start capturing audio again allowing you to add missing information, continue on a previous encounter, or do your encounter (if you like to pre-generate notes and then do the encounter). A real time transcript shows what you are saying and simply click Conclude once done. Autochart.ai will automatically take the addendum and regenerate the note with the added information. What you say is also shown at the bottom of the Transcription Tab.

The Notes View allows you to review, edit, and manage your AI-generated medical notes with options like formatting, email sharing, archiving, and regenerating for customized outputs. Remember to transfer records to your EMR promptly, as charts are stored for 30 days for privacy and security.
