Encounters Overview

An encounter is one patient visit. Autochart.ai listens to the conversation, then writes a clinical note you can review, edit, and copy into your EMR. From the same visit you can also generate patient letters, referrals, handouts, and more — all without re-entering anything.

How to access: Left sidebar -> Encounters. It expands into four lists.

| List | What it holds | | --- | --- | | Completed | Visits with a generated note ready to review and copy. | | Scheduled | Upcoming visits booked for a patient. Requires Patient Management. | | Saved | Recordings saved with Save for Later before a note was generated. | | Incomplete | Recordings that were started but the browser was closed before generating a note. |

The basic workflow

  1. Click New Encounter in the left sidebar.
  2. Adjust Encounter Settings if needed (note type, language, specialty).
  3. Record the visit.
  4. Click Generate Note.
  5. Review and edit the note in Completed, then copy it to your EMR.

What you can do from Completed

Click any encounter in Completed to open its note. From there you can:

  • Edit the note directly or use Chat to ask the AI to make changes.
  • Copy the note in rich text or plain text for pasting into your EMR.
  • Switch to Transcript, Patient Communication, Provider Communication, or Resources using the tabs on the right.
  • Regenerate the note with different settings.
  • Archive or delete the visit from the three-dot menu.

Tips

  • Saved vs Incomplete: Saved recordings sync across devices. Incomplete recordings are stored only on the device and browser where they were started — clearing browser data removes them.
  • Retention: Visits are removed automatically based on your Data Retention settings. See Settings -> Data Retention.
  • Bulk actions: On the Completed page, tick multiple visits to archive or delete them at once.
