Adding a patient record lets you link their encounters to a profile, so Autochart.ai can reference their name, date of birth, and visit history when writing notes. It also lets you schedule visits ahead of time.
How to access: Left sidebar -> Patients.
Plan availability: Patient Management is included on the Intensive plan or higher. On Starter and Daily it is available as an add-on. See pricing and plans.
Steps
- Go to Patients in the left sidebar.
- Click Add Patient at the bottom of the page. You can also click Add New Patient from the scheduling dialog.
- Fill in the required fields: First Name, Last Name, and Date of Birth.
- Fill in any optional fields you want: Gender, Sex, Pronouns, Phone Number, Email, Medical Record Number (MRN), and Personal Health Number (PHN). These can all be added or updated later.
- Click Save.
The patient now appears in your Patients list. You can link future encounters to their record or schedule visits directly from their row.
Tips
- You can add a patient on the fly while scheduling — click Add New Patient in the scheduling dialog rather than coming to this page first.
- The MRN and PHN fields are optional but useful for matching records with your EMR.
- Duplicate names are allowed. If two patients share a name, use their date of birth, phone number, or MRN to tell them apart in the list.
Troubleshooting
- Save button is greyed out: First Name, Last Name, and Date of Birth are all required.
- Patients page is missing: Enable Patient Management in Settings -> Patient Management. Your plan or an add-on needs to include it.
- Import from CSV: Coming soon — not yet available.
